  • Pay stubs or other documents evidencing your income for the past 90 days. This includes any non-wage income such as social security, public assistance, spousal maintenance, trust account and retirement account funds.
  • Copies of your tax returns for the past 3 years.
  • Most recent statements from all your creditors, which includes their addresses, amounts still owing, and account numbers.
  • Bank statements for the past 90 days.
  • A list of your assets including:
    • Automobiles, including amount owed and market value
    • Tools of your trade, if any
    • Jewelry and value
    • Life insurance policies
    • Retirement accounts such as an IRA, 401K, or pension
    • Antique or items of unusual value
    • Firearms
    • Electronics
    • Boats
    • Real estate
    • Checking and savings account numbers and amounts
    • Certificates of Deposit (CDs) and annuities
    • Stocks
    • Money that is owed to you, including tax refunds
  • Make a list of your monthly expenses.
  • Driver's license or government-issued photo ID.
